Your pushing away from your body and flicking your wrist. At your height, that type of motion is causing the ball to gain too much potential energy after leaving Point A (your hand) resulting in too much kinetic energy at Point B (the backboard/backside of the rim). A smooth follow through with your wrist causing the ball to roll off instead of being flicked out will create a nice smooth arch that will cause the ball to sail smoothly through the hoop. Another tip is to focus on the empty space created by the shape of the hoop. It's a little more difficult at your height since the empty space will be a lot smaller compared to what a 5'10 guy like myself sees but keeping your eye trained on that empty space creates an automatic path to the basket.
